The delusional misidentification syndrome (DMS) has been associated wi
th a range of neurological conditions. Three cases of DMS in patients
with Parkinson's disease and dementia, treated with dopaminergic medic
ations, are presented. It is postulated that DA IS associated with par
kinsonism results from a combination of dopaminergic psychosis and cog
nitive dysfunction involving the frontal lobe in particular DMS in the
setting of parkinsonism may be move frequent than commonly supposed.
