We assessed whether there are differences among patients with inferior acut
e myocardial infarction and ST segment depression in leads V2-V3 between th
ose with positive versus negative T waves on admission. We found no differe
nces in the prevalence of concomitant significant left anterior descending
coronary artery stenosis and anterior or posterior wall motion abnormalitie
s on echocardiography between the two groups. Precordial ST segment depress
ion with negative T wave may be the early electrocardiographic pattern of p
osterior infarction, reflecting ST segment elevation with positive T waves
that would have been detected by leads facing the posterior wall.
