Clozapine, an atypical antipsychotic, is mainly approved for the treat
ment of resistant schizophrenia. However, a substantial body of eviden
ce suggests that it might be useful in other psychiatric indications,
such as treatment-resistant depression, Parkinson's disease, and demen
tia. In this report we present the cases of three patients hospitalize
d at the psychiatric division of the Sheba Medical Center, diagnosed w
ith major depressive disorder with cognitive impairment, whose present
ing symptom was agitation. These patients were nonresponders to variou
s treatment modalities. However, treatment with clozapine brought abou
t a favorable response.
