A new case of locally migrating reflex sympathetic dystrophy syndrome
(RSDS) is reported. Two episodes occurred in the same foot a few month
s apart, one in the cuneometatarsal region of the first ray, and the o
ther in the metatarsophalangeal region of the second ray. The first ep
isode had resolved when the second episode occurred. The diagnosis wit
h exertion-induced acute transient bone insufficiency fractures can be
difficult, especially as these fractures can be complicated by RSDS,
and vice versa. The migratory pattern of RSDS was first identified bas
ed on bone scan findings, which allowed to differentiate it from true
recurrent RSDS and to demonstrate that the latter is exceedingly rare.
