Needlescopic surgery is a refinement of laparoscopic surgery wherein i
nstruments smaller than 3 mm are used. The advantage is the minute wou
nds that heal imperceptibly. Initial reports of needlescopic cholecyst
ectomy were confined to uninflamed gallbladders where the antegrade te
chnique was invariably used. This case demonstrates a needlescopic cho
lecystectomy by the retrograde approach for an inflamed gallbladder.
