Objective: Determination of fetal fibronectin (fFN) levels in a patien
t at risk of preterm delivery and having premature rupture of the memb
ranes. Patient and Methods: The present case report concerns a pregnan
t woman with premature rupture of the membranes at 16 weeks of gestati
on who was submitted to repeated therapeutic transabdominal amnioinfus
ions and had a spontaneous resolution of the rupture at 32 weeks of ge
station. Results: A good pregnancy outcome was observed. High cervicov
aginal fFN levels were observed during the period of membrane rupture
with following drop of fFN levels during this period and spontaneous r
eseal of the membranes. Conclusions: Active expectant management, usin
g repeated amnioinfusions, probably played a useful role in the therap
eutic success of this pregnancy. Further, fFN was a reliable marker of
the rupture of membranes. The possible role of fFN in the clinical ma
nagement of high-risk pregnancies is confirmed.
