The authors report the case of a 25 year old man with unexplained pulm
onary hypertension during his first hospital admission. However, a pas
t medical history suggesting right arm vein thrombosis was obtained. V
enography showed signs of thoraco-brachial compression. After heart-lu
ng transplantation, pathological studies of the explanted organs showe
d changes of thrombo-embolic pulmonary hypertension. A spontaneous rig
ht arm venous thrombosis occurred during convalescence. The possibilit
y of post-embolic pulmonary hypertension caused by venous thrombosis o
f the upper limbs is discussed.
