We describe the case of an ectopic pregnancy that implanted in the ute
rine scar five years after a Caesarean section. The diagnosis was made
by vaginal and duplex sonography after measurement of a HCG value of
19748 IU/ml. After a single intramuscular injection of methotrexate (1
mg/kg body weight) the pregnancy was dissolved. The patient was follo
wed up by weekly HCG determination and several ultrasonographic contro
ls until complete normalisation. Thanks to this conservative treatment
the potential fertility of the patient was preserved and the complica
tion of uterine rupture could be avoided.
