There have been impressive developments in the pharmacological treatme
nt of mood disorders in the elderly, both acute and long-term, and in
the management of treatment-resistant depression. The nihilism that ha
d prevailed previously has been replaced by cautious optimism with reg
ard to the results of controlled trials in naturalistic settings and s
tudies in high-risk groups, including patients with multiple medical c
