H. Chataigner et M. Onimus, L5 SCIATICA DUE TO PRESACRAL COMPRESSION OF THE LUMBOSACRAL TRUNK - ACASE-REPORT, Revue de chirurgie orthopedique et reparatrice de l'appareil moteur, 84(4), 1998, pp. 377-380
A 42 years old patient presented with a 4 years history of a left L5 s
ciatica unresponsive to a previous L4-L5 surgical decompression. CT sc
an showed a solid tumor developped on the anterior aspect of the sacro
-iliac joint, in contact with the lumbo-sacral trunk. An antero-latera
l extraperitoneal approach was performed, with progressive subperitone
al dissection along the inner aspect of the iliac wing to the sacro-il
liac joint and sacral ala. The exostosis was removed with ''en block''
excision Postoperatively pain completely disappeared. Considerations
are made on the mechanism of the compression as well as on the surgica
l approach performed.
