A 14-year-old girl with perinatally acquired human immunodeficiency vi
rus infection had fatal intravascular hemolysis after intravenous admi
nistration of ceftriaxone. Laboratory studies confirmed the presence o
f an antibody against ceftriaxone in the serum and on the patient's re
d blood cells. No evidence of sepsis, glucose-6-phosphate dehydrogenas
e deficiency or anaphylaxis was found.
