We report the cases of two patients with cerebral embolization and int
eratrial septal aneurysm. Transesophageal echocardiography required fo
r stroke showed right-sided interatrial septal aneurysm in the first p
atient and left-sided interatrial septal aneurysm in the second. In th
e two patients, interatrial septal aneurysm was mimicking atrial cysti
c tumor or abscess. Significant internal carotid artery stenosis was f
ound in the first patient and aortic arch atheroma with mobile compone
nts but no patent foramen ovale in the second patient. In the two pati
ents, interatrial septal aneurysm appears to be only an incidental fin
ding and not the true cause of stroke. Furthermore, we discuss the dif
ferential diagnoses of atrial masses.
