The therapy of atopic eczema remains a challenge. The success of any t
herapeutic concept is based on an broad and early diagnostic approach
which allows to rule out relevant provocation factors and allergens. D
uring remission periods the regular use of a topical basic therapy con
sisting of drug-free emollients is recommended. Topical corticosteroid
s as well as systemic or local antimicrobial therapy and antihistamine
s are essential during periods of acute exacerbations. Although during
the last years a great number of new therapeutic approaches have been
published, data of most of these therapeutic modalities are not suffi
cient to allow an unrestricted use in all patients with atopic eczema.
