A 14-year-old boy presented with anorexia and weakness whereon the dia
gnosis of dimorphic anaemia was made. An excellent response to iron an
d vitamin B-12 was observed. In addition, the patient had non-obstruct
ive hypertrophic cardiomyopathy. At endoscopy 2.5 years later, an aden
ocarcinoma was diagnosed and the patient underwent a high subtotal gas
trectomy. To the best of our knowledge, this rare association has neve
r been reported in children. Conclusion We report a youngster with per
nicious anaemia, associated with non-obstructive hypertrophic cardiomy
opathy in whom gastric adenocarcinoma was found. Patients with pernici
ous anaemia are at greater risk of developing gastric carcinoma than t
he general population, therefore we recommend routine periodic gastros
copic surveillance in the paediatric population with pernicious anaemi
