Acute tubulointerstitial nephritis associated with uveitis is an infre
quent entity. The etiology is unknown but in its pathogenesis, cellula
r immunological mechanisms have been implicated. We describe of a thir
teen-year-old girl who presented with a constitutional syndrome and ac
ute renal failure (serum creatinine 2.2 mg/dl) with a later developmen
t of an anterior uveitis. The pathology showed an acute tubulointersti
tial nephritis with a predominant T lymphocytic infiltrate. The immuno
fluorescence was negative. The patient was treated biochemical with co
rticosteroids which produced dramatic clinical and biochemical improve
ment. We emphasize the importance of diagnosing this syndrome, the rol
e of kidney biopsy, and the successful steroid treatment.
