A 49-year-old woman had large annular lesions with raised erythematous
borders and atrophic, hypopigmented central regions on her neck, trun
k, and proximal upper extremities. Examination of a biopsy specimen de
monstrated a granulomatous infiltrate in the upper dermis that surroun
ded areas lacking elastic fibers; elastic fibers were present within t
he multinucleated giant cells. The clinical presentation and pathologi
c findings were consistent with a diagnosis of annular elastolytic gia
nt cell granuloma (AEGCG). The clinical description and pathologic dif
ferential diagnosis of AEGCG are reviewed.
