This summary provides an overview of the assessment and treatment reco
mmendations contained in the Practice Parameters for the Assessment an
d Treatment of Children and Adolescents With Depressive Disorders. Dep
ressive disorders in children and adolescents are marked by core sympt
oms similar to those seen in adults, although symptom expression varie
s greatly with developmental stage. These disorders are common, chroni
c, and recurrent, and they are associated with comorbid psychiatric co
nditions and poor outcome that can be alleviated by early identificati
on and treatment. Opinions differ regarding treatment planning and dur
ation of treatment required. Development of a treatment relationship w
ith the patient and family is crucial for a successful outcome. Psycho
therapy is an appropriate treatment for all children and adolescents w
ith depressive disorders. Antidepressants may prove useful in some cas
es and are especially recommended for patients with psychosis, bipolar
depression, and severe depression. Continuation treatment is necessar
y in all patients after the acute phase; long-term maintenance is requ
ired in some. Early detection and intervention is effective in amelior
ating the poor psychosocial outcome.
