Bupivacaine is a commonly used local anesthetic in dental, ophthalmolo
gic, and simple surgical procedures. Its current popularity derives fr
om its potency and relatively long half-life. Widespread use of bupiva
caine has resulted in sometimes severe adverse reactions when signific
ant systemic absorption has occurred. This report documents a life-thr
eatening event following use of bupivacaine, briefly reviews its neuro
toxic and cardiotoxic effects, and raises questions about current mana
gement strategies when toxicity occurs.
