Leukocytoclastic vasculitis (LV) is a pathologic finding in many disor
ders, including infection, malignancy, and systemic vasculitides. We d
escribe a patient with palpable purpura resulting from LV, inflammator
y polyarthritis, mononeuritis, and a positive anti-neutrophil cytoplas
mic antibody (ANCA) who was found to have a renal cell carcinoma. Afte
r removal of the carcinoma, the symptoms resolved and have remained ab
sent for 1 year. To our knowledge, this is the fifth case report of LV
as the presenting feature of renal cell carcinoma and the first to in
clude systemic vasculitis, an inflammatory polyarthritis, and a positi
ve ANCA.
