Topiramate is a sulfamate-substituted monosaccharide that has demonstr
ated efficacy as an antiepileptic drug in adults with partial onset se
izures. Experience in children has been limited, but early reports hav
e supported its safety and effectiveness in children as young as 2 yea
rs of age. In two infants ages 12 and 9 months, respectively, with par
tial seizures, the authors report excellent efficacy with good tolerab
ility at doses up to 7.7 mg/kg, Although long-term safety and possible
adverse sequelae have not been fully established in children, topiram
ate may represent an option for infants with high seizure frequency un
responsive to standard antiepileptic drugs. (C) 1998 by Elsevier Scien
