We describe two patients who first presented with congenital muscular
torticollis (CMT) at ages of 32 and 47 years, respectively. Characteri
stic features included fixed torticollis since neonatal period, ineffe
ctiveness of sensory tricks, cord-like consistency particularly of the
clavicular portion of the sternocleidomastoid muscle on palpation due
to fibrosis that can be visialized on magnetic resonance imaging. Car
eful history taking and documentation of abnormal head postures on chi
ldhood photographs may point to CMT that is to be included in the diff
erential diagnosis of non-dystonic torticollis in adults. There is no
indication for local injections of Botulinum toxin in this condition.
