Pneumococcal pericarditis is usually treated by surgical drainage and
antimicrobial therapy. In a 60-year-old man with tamponade due to pneu
mococcal pericarditis, a drain was inserted percutaneously under the x
yphoid process using ultrasound guidance. The drain was left in place
for 11 days. Antimicrobials were given systemically and injected into
the pericardial cavity. The immediate and five-month outcomes were fav
orable. The respective roles of medical and surgical therapy are discu
ssed.
