A 64-year-old man with common variable immunodeficiency developed a persist
ent papulonodular ulcerative eruption on the right leg. Histopathological e
xamination disclosed a chronic inflammatory infiltrate with central necrosi
s and palisading granuloma. Repeated microbiological (bacteriological, myco
logical and mycobacteriological) studies failed to isolate any microorganis
m. After treatment with intravenous immunoglobulins, a progressive resoluti
on of the skin lesions was observed with a complete clearing after 10 month
s. Clinicopathological features and therapeutic approaches of sterile granu
lomatous lesions associated with primary immunodeficiencies are reviewed.
