Noncontiguous multidermatomal herpes tester is very rare in both immunocomp
etent and immunocompromised persons. Most of the reported cases have been l
imited to 2 noncontiguous dermatomes. This unique presentation has been ref
erred to as tester duplex uni lateralis or bilateralis, depending on whethe
r one or both halves of the body are involved. Granulomatous dermatitis at
sites of herpes tester scars, a rare isotopic response, has only been repor
ted in persons with contiguous dermatomes of tester. We describe an immunoc
ompromised patient who developed herpes tester in 7 disparate dermatomes. T
hree months after resolution of the tester, the patient developed a granulo
matous dermatitis in a zosteriform distribution at the sites of previous in
fection.
