We herein report the case of a patient with idiopathic thrombocytopenic pur
pura (Werlhof disease) and coronary artery disease undergoing myocardial re
vascularization. The use of monomeric immunoglobulins, corticosteroids, pla
telets transfusion, use of a cell saver, normothermic cardiopulmonary bypas
s, aprotinine and homologous blood transfusion were combined in order to mi
nimize the risk of bleeding complications in the postoperative period.
