Practice guidelines such as those of the United States Public Health Servic
e Agency for Health Care Policy and Research have been instrumental in addr
essing the significant problem of how best to manage major depression in pr
imary medical care settings. Since this set of guidelines was published in
1993, new findings from randomized clinical trials and extensive clinical e
xperience permit us to reevaluate trends in treatment of major depression i
n primacy medical care. This review suggests guidelines for achieving best
clinical practice given current knowledge.
