A 21-year-old developed progressive headaches and pulsatile tinnitus. He wa
s found to have papilledema and a pulsatile bruit. A dural arteriovenous ma
lformation was not recognized on brain CT or MRT, but was well documented o
n magnetic resonance angiography and cerebral angiography. Dural malformati
ons draining into the superior sagittal sinus can cause increased intracran
ial venous pressure and secondarily increase intracranial pressure even wit
hout a mass effect.
