We report three cases of postoperative recurrent cholangitis due to a defec
tive hepaticojejunal anastomosis. Causal diseases were alveolar echinococco
sis of the liver alcoholic chronic pancreatitis, liver colorectal metastase
s. Clinical presentation included major cholestasis and cachexia. Imaging e
xplorations showed that cholangitis was due to an inversion of the Roux-en-
Y jejunal loop which had been disposed in a wrong position. Clinical improv
ement was remarkable after reoperation and replacement of the defective loo
p in the right position. This exceptional cause of postoperative cholangiti
s after Roux-en-Y hepaticojejunal anastomosis must be identified and treate
d by prompt restorative surgery.
