Syphilitic gummas have become exceedingly rare in present practice. Approxi
mately one-third of untreated cases of syphilis result in tertiary syphilis
which can be subdivided into benign syphilis (gumma), cardiovascular syphi
lis or neurosyphilis. Gumma represents an inflammatory process and heals wi
th scarring. It can heal spontaneously but recurrence in untreated or inade
quately treated cases can occur in the same or another site. Resolution is
prompt under treatment, although recurrence is not inevitable. We report an
interesting case of basal cell carcinoma (BCC) that developed at the site
of a previous gumma and discuss the possibility of a causal association bet
ween both conditions.
