Replacement lipomatosis of the kidney is the result of severe atrophy or de
struction of the rrenal parenchyma often caused by calculous disease with s
econdary marked proliferation of renal sinus, renal hilus, and perirenal fa
tty tissue. The diagnosis is difficult to establish with conventional radio
graphic methods. Although ultrasonography may show highly;suggestive findin
gs, computed tomography seems to be the most accurate method for demonstrat
ing the distinctive features of replacement lipomatosis. Uultrasonographic
and computed tomographic features in three cases of replacement lipomatosis
of the kidney are reported.
