Cystic dilatation of bulbourethral gland ducts is a rare pathology known as
Cowper's syrincocele. It is normally found in children and young adults. W
e report a case of late clinical manifestation of a large dilatation of Cow
per's gland duct presenting as, a perineal cyst. Diagnostic procedures cons
isted of ultrasonography, urethrography and urethrocystoscopy. Magnetic res
onance imaging proved to be an excellent tool to demonstrate the topographi
c relationship between the syringocele and the urethra and corpus spongiosu
m. The syringocele was treated successfully by open perineal excision. As t
he syringocele seems to be more common than previously reported, the physic
ian should consider it in patients with unusual genitourinary symptoms.
