Prior meetings of the International Conference on Neoadjuvant Hormonal Ther
apy in Prostate Cancer identified areas of concern in the pathologic analys
is of post-treatment specimens. In response, a pathology subcommittee was f
ormed to address the questions that had been raised. Recommendations are ma
de here concerning pretreatment biopsy review and reporting, handling and r
eporting of radical prostatectomy specimens, Gleason scoring of treated can
cer, grading of therapy effect, role of immunohistochemistry procedures, an
d the significance of high-grade prostatic intraepithelial neoplasia. Neoad
juvant trials with both surgery and radiation therapy were considered in dr
awing up these recommendations.
