Case report on a patient who suffered on right-sided C6 root compression du
e to prolapsed intervertebral disc C5/C6 mediolaterally. Directly after ope
ration (Cloward) we found a severe pure motor deficit of the right hand. Cl
inical investigation and neurophysiological data showed a nuclear type lesi
on of segment C8/D1, but improvement of C6-lesion. T2-weighted MR-image sho
wed a hyperintense spot in the lower cervical cord (C8/D1) in right-hand an
terior horn area, We conclude, that this lower motor neuron lesion is cause
d by spinal cord infarction in the anterior horn area, Intraoperative lesio
ns of radicular artery feeding anterior spinal artery may cause infarction
within the territory of the sulcocomissural artery.
