Paraesophageal omental herniation (POH) is uncommon. CT scan and MRI are co
mplementary in diagnosis. We present a posterior mediastinal mass in a 43 y
ear old male with a history of myxoid liposarcoma raising the suspicion of
latent secondary tumor. Subsequently, at thoracotomy he was found to have a
POH. Differential diagnosis, work-up and surgical approach are discussed.
