During the past 5 years, the authors have used a direct trans-lower lid ble
pharoplasty subperiosteal approach to the lower lid and midface for the pur
pose of correction of midfacial aging in 757 patients. In a smaller but sig
nificant group, this approach has proven valuable in difficult reconstructi
ve situations. The purpose of this article is two-fold: (1) to provide a co
mprehensive retrospective evaluation of the value and promise of the techni
que and (2) to provide a comprehensive discussion of the pitfalls and compl
ications that have been associated with use of this technique. In addition,
technical modifications that may lower the rare of morbidity associated wi
th the use of the procedure are described.
