In an attempt to identify a focus of infection, WBC scintigraphy was p
erformed, and it showed increased accumulation in the midanterior pelv
is suspicious for a site of infection. Computed tomography of the pelv
is obtained after WBC scintigraphy showed gross retention of stool and
a distended bladder, hut no suggestion of infection. Repeat WBC scint
igraphy performed with a Foley catheter in peace after bowel cleansing
did not show any abnormal tracer accumulation. This case shows how th
e combination of a distended bladder and fecal retention can produce i
mages suggestive of a site of infection.
