Severe acute mitral regurgitation (MR)can occur with ruptured whole pa
pillary muscle or isolated heads of papillary muscles or chordae tendi
nae during the course of myocardial infarction.(1,2) 2 Furthermore MR
may be precipitated by impaired function of non-ruptured papillary mus
cle after myocardial infarction.(3) We report an unusual case of trans
ient severe MII, as evidenced by transient giant 'V' waves in the dire
ct left atrial pressure (LA) tracing during angina pectoris.
