Adrenocortical carcinoma manifesting pure hyperaldosteronism is extremely r
are. We report here a 61-year-old woman with biochemically proven primary a
ldosteronism due to right adrenocortical carcinoma. Computed tomographic sc
an showed 4.5x5.3 cm lobulated mass with tiny calcification, while there wa
s no significant uptake of I-132-iodomethyl norcholesterol in the tumor. Im
munohistochemical analysis demonstrated expression of steroidogenic enzymes
in the tumor tissue: P-450(scc), P-450(c21), 3 beta-hydroxysteroid dehydro
genase, P450(17 alpha), and P-450(11 beta). In addition, we could demonstra
te mRNA expression of aldosterone synthase (P-450aldo: CYP11B2) in the tumo
r by specific ribonuclease protection assay. This is the first report of a
case of primary aldosteronism due to adrenocortical carcinoma, in which exp
ression of all sets of steroidogenic enzymes required for aldosterone synth
