We report a case of distal end malfunction in a child with ventriculoabdomi
nal shunt. The distal placement was in the gallbladder rather than the peri
toneal cavity, as is usually the case with a ventriculoperitoneal shunt. Su
rgeons should be alerted to the possibility of distal terminus of a shunt t
o be other than the peritoneal cavity, as revision surgery in patients with
ventriculogallbladder shunt requires exposure of the metal connector on th
e gallbladder wall to prevent biliary leakage.
