Acute renal insufficiency in the course of pyelo-ureteral junction (PUJ) sy
ndrome in a single kidney is a rare occurrence. The authors report on an un
common case of a 66-year old woman who underwent a right nephrectomy for py
elonephritic lithiasis, and who was subsequently admitted with acute renal
insufficiency and left hydronephrosis determined by ultrasonography, After
failure for upper ureteral stent, pyelography carried out via a nephrostomy
confirmed left PUJ syndrome, The follow-up was characterised by the worsen
ing of renal function. Following hemodialysis, surgical investigation deter
mined the presence of a giant hematoma with PUJ stenosis, Anastomotic Ander
son-Hynes-type resection was carried out with the insertion of a double J u
reteral stent. The follow-up was uneventful, with normalisation of renal fu
nction at day 6 post-surgery and removal of the ureteral stent one month la
