In a hemodynamically compromised neonate with complete transposition and in
tact ventricular septum (concordant atrioventricular and discordant ventric
ulo-arterial connections), it became necessary to control severe congestive
heart failure. The arterial switch operation had been delayed because of i
ntracranial bleeding and subsequent coagulopathy. The cardiac failure was r
eversed by surgical banding of the patent arterial duct. The arterial switc
h was performed successfully one week later.
