Abstract
This article reports on the development and initial implementation of a sel f-report outcomes measurement instrument. The instrument, tested with veter ans in programs at four Veterans Administration Blind Rehabilitation Center s, provided assessments of their functioning on 32 personal and social acti vities at the beginning and near the end of their programs.
