Bilateral adrenal haemorrhage of traumatic origin is rarely observed or pos
sibly missed in severely multi-traumatised patients. It can lead to a poten
tially fatal adrenal shock. its emergency diagnosis is made by imaging tech
niques, usually by CT-scan. Early substitution therapy has to be done. This
complication emphasizes the importance of an immediate abdominal morpholog
ical exploration in multi-traumatised patients when this is feasible. (C) 2
