Objective To review a case of cerebellar infarction after uncomplicated lap
aroscopic hysterectomy.
Methods The clinical course and radiological findings in a 45-year-old prev
iously healthy woman are reviewed.
Results The patient suffered from headache and dizziness from the first pos
toperative day after a surgically uncomplicated laparoscopic hysterectomy.
At 1 week after the operation she was readmitted because of continuing symp
toms. Computer tomographic scan results of the brain were normal, while mag
netic resonance imaging revealed two small infarctions in the right cerebel
lar hemisphere. At 3 years after the operation she still has mild neurologi
cal symptoms and is not capable of her previous work.
Conclusion Haemodynamic changes related to head-down tilt position combined
with pneumoperitoneum may have hazardous effects on the brain even in prev
iously clinically healthy patients.
