Percutaneous and limited open fixation of fractures of the distal radius is
an important method of treatment for many unstable fractures such as unsta
ble dorsal bending fractures, shearing fractures of the radial styloid and
lunate facet, and simple articular fractures. The quality of the reduction
is monitored with image intensification and the tactic of the reduction is
based on manipulation of the fracture fragments by longitudinal traction, p
ercutaneous manipulation, and in some instances by direct manipulation thro
ugh small incisions. The role of arthroscopy remains uncertain and may repr
esent an alternative to open exposure of the articular surface in some pati
ents although bone grafting may be necessary in patients with fractures wit
h significant metaphyseal defects.
