In the last few years an increase in the number of candidates for liver tra
nsplantation has been observed, However the donor pool has not increased pr
oportionally so that the lack of available donor organs remain a major conc
ern. Living-related liver transplantation is actually one of the strategies
to maximize donor organ use not only for paediatric but also for the adult
patient population. The authors report their experience with the first adu
lt-to-adult living-related liver transplantation using the right lobe. Desp
ite a donor portal anomaly, the donor operation and the transplantation wer
e uneventful. After six months' follow-up, donor and recipient are in excel
lent clinical state.
