An unusual case of isolated levocardia, duodenal obstruction, and malrotati
on with volvulus is reported. Three cases of situs inversus abdominus with
malrotation with volvulus have been reported to date. The current case is t
he fourth case and only the second survivor in the series. In cases of situ
s inversus abdominus, because of inversion of viscera, the volvulus may occ
ur in an anticlockwise direction and, thus, should be derotated in a clockw
