Aim. To study antihypertensive efficacy and tolerability of angiotensin con
verting enzyme inhibitor trandolapril. Material. Patients with stage I-II (
WHO classification) essential hypertension (n=34, 18 men and 16 women, mean
age 49,6+/-8,78 years). Control group comprised 20 healthy persons. Method
s. Echocardiography and 24-hour blood pressure monitoring were carried out
before and in 4 weeks of treatment. The dose of trandolapril depended on in
itial blood pressure and was 0,5-4 mg o.d. Results. Treatment with trandola
pril was associated with significant reduction of total peripheral vascular
resistance by 16,8%, and lowering of mean 24-hour blood pressure by 10,2%.
According to results of blood pressure monitoring high hypotensive efficac
y of trandolapril was sustained throughout 24 hours. Daytime blood pressure
was lowered by 10,9% (systolic) and 9,4% (diastolic), nighttime blood pres
sure - by 7,3% (systolic) and 8,8% (diastolic) (p<0,05). Portion of measure
ments with figures above normal also substantially decreased. Moreover tran
dolapril affected favorably parameters of circadian blood pressure rhythm.
Efficacy of the drug was increased by concomitant use of diuretics and calc
ium antagonists from phenylalkylamine group. Conclusion. In patients with e
ssential hypertension trandolapril exhibited high efficacy and good tolerab
ility.
