The primary significance of traumatic disruption of the scapholunate ligame
nts is alteration of radioscaphoid joint mechanics. The term rotary subluxa
tion of the scaphoid best describes this condition and was first identified
more than 50 years ago. With disruption of the critical ligamentous link b
etween the lunate and scaphoid, the distal scaphoid flexes and the proximal
pole subluxates dorsally toward the distal radius rim. This article presen
ts the authors' current approach to diagnosis and treatment of rotary sublu
xation of the scaphoid.
