The frontalis muscle and its fascia are connected with the orbicularis ocul
i muscle at the level of the eyebrow. Therefore, the superiorly based orbic
ularis oculi muscle flap, when advanced and attached to the tarsal plate, c
an dynamically elevate the upper lid and correct blepharoptosis with previo
us frontalis sling. Six patients with undercorrected blepharoptosis after f
rontalis sling suspension were included in this study. The superiorly based
orbicularis oculi muscle flap was advanced to the tarsal plate in these pa
tients and the redundant portion of the distal flap was resected. Postopera
tive results were satisfactory after 1-year follow-up. (C) 2000 The British
